What is post-hospitalization care?

Post-hospitalization care, in the context of non-medical home care, refers to the continuation of personal care and daily living assistance for clients who already had home care services in place before a hospital stay. When these clients return home, their existing non-medical care plan resumes — covering help with bathing, dressing, meals, light housekeeping, and daily routines — so life at home continues with as little disruption as possible.

Is this the same as skilled nursing or therapy services after a hospital stay?

No. Stay At Home Homecare provides non-medical home care only. Skilled services — such as visits from a nurse, physical therapist (PT), or occupational therapist (OT) approved through Medicaid following a procedure or surgery — are provided by separate, licensed agencies. Our role is to support the client at home with daily non-medical tasks, not to provide or replace those clinical services.

Do you only serve clients who already had care with you before the hospital stay?

Our transitional care support at home is primarily for clients who had a home care plan in place with us prior to hospitalization and are simply resuming that care. If you are new to home care and returning home from a hospital stay, we encourage you to call and speak with a coordinator to discuss your options.

How soon can care resume after a client comes home?

We work with families to have care in place as quickly as possible. A care coordinator can discuss what is needed and confirm a schedule before your loved one is discharged, so support is ready from day one at home.
